Older adolescents leaving foster care still face tremendous obstacles as they make the transition to adulthood. However, the increasing focus on improving foster care and permanency planning for adolescents, providing them with independent living skills, and giving them the support that they need as they make the transition out of care is cause for optimism. This article highlights programs that are a few examples of innovative solutions that communities are creating and enacting in efforts to change the tide.
